There are several ways to decide which family member is 1st, 2nd, or 3rd+ in a Family Care Plan. However, the method you choose should be standardized, applied to all patients on a care plan, and should be documented in your compliance manual (even if you are a CHUSA provider).

We suggest the following:

The family member with the highest number of visits during the treatment plan is listed as #1, followed by the next-highest number of visits as #2, #3, etc.

What happens if one family member drops out of care?

You will create a new plan for the remaining family members and adjust their position based on the method above.


Example methods that do not work with the Care Plan Calculator® System:

  1. The position of each family member is determined by the date of their first visit.

  2. The position of each family member is determined only if they come together on the same day/same week.
